Here's an update that now includes a description of loader files. I'd really like to hear what people think about this doc, areas that need improvement or sections that need adding.

Well, here's the final version. I think I've done all I can with it, feel free to edit and distribute.

This seems much easier to parse than the old manual, thanks for all your work on this!
