I noticed that you set the RESET vector to $AFA2 but your patch is actually at $AFAE, was there a reason for that?

Sounds like you have either a stuck C= (commodore key) or you held the number 6 key once when you booted up.

With the MC inserted try holding the number 0 key then power up, that should reset to the main menu.

You might also try holding the CTRL key on power up, and select "RESET PREFS".

eslapion is correct.
